Golden Years Transitional Care Unit is a 20-bed unit specializing in rehabilitation and the medically complex patient. The primary focus is to enhance healing through a multidisciplinary approach to care. Patients who require more then routine care with intensive therapies and nursing care may need the specialized serves that the Transitional Rehab Unit can provide. We specialize in caring for Clinically Complex patient with prolonged surgery recovery or intensive care. Our Wound Care Program is designed for patients with impaired tissue such as decubitus ulcers and burns, or who are at high risk for complications from amputation. Individual Rehabilitation is designed specifically for each patient requiring therapy utilizing mobility, strengthening endurance, retaining the activities of daily living and dysphasic diagnosis and treatment. Rehabilitation therapy services are most often prescribed for, but not limited to, the following conditions: Arthritis, Stroke, Orthopedic complications, Major trauma, Post-cardiac conditions, Fractures, Hip/knee replacements, Head injury, Degenerative diseases
