Prior Admission
The process of admission to any senior rehabilitation and long-term care center can be overwhelming. If you desire, one or more members of our friendly team of professionals will meet you at the hospital or your home to address any concerns you may have about your care or that of your loved one. We will also provide you with a list of items to bring to the center.
Admission
Our admissions director will sit with you and/or your responsible party to fill out the required forms and discuss the payment options available to you. To aid in the process, we kindly ask that you bring copies of all of your insurance card.
Plan of Care
During your stay at our senior nursing home in Baltimore, the interdisciplinary team will meet to discuss the individualized plan of care for you or your loved one. You and/or your family are always invited to attend these meetings, which address both physical and emotional needs. Progress reports can be provided to personal physicians to ensure continuity of care. Family collaboration allows us to provide the best set of senior care services for your loved one.
