Tours of new specialty unit at Grand Opening Event

PARMA HEIGHTS, Ohio (July 19, 2017) – Greenbrier Healthcare Center, part of the Communicare Family of Companies, has created a new specialty Transitional Care Unit for post-hospitalization cardiac and pulmonary care, in partnership with University Hospitals.
Tours of the new unit will be available at a Grand Opening event on Tuesday, July 25, 4 – 7 p.m at Greenbrier Healthcare Center, 6455 Pearl Rd., Parma Heights. There will be hors d’oeuvres and musical entertainment by The John Pearson Incident.

This new program, developed by a team of heart and vascular experts from University Hospitals, provides innovative and high-tech care for hospital patients recommended for rehabilitation after a cardiac or respiratory surgical procedure, illness or injury. It features the use of technology such as EKGs and telemetry, and extensive cardiologist involvement, in addition to a smooth, detailed transition between cardiology hospital staff and the highly-trained healthcare center staff at Greenbrier. No other skilled nursing centers within 12 miles of Greenbrier have EKG and telemetry.

This program allows Greenbrier to take patients with complex medical needs from the hospital with seamless communication and transition between UH specialists in the hospital and the clinical team at Greenbrier.

This transitional care unit – which offers a higher ratio of staff to patients – is expected to result in lower re-admission rates, meaning less chance of returning to the hospital during a rehab stay because of unexpected complications. Reducing the return to hospital matters because it allows patients to heal faster and return home sooner.  The goal is to achieve quality care in the least restrictive environment.

Greenbrier continues to grow these kinds of collaborative relationships to meet the needs of the community.

University Hospitals has had the program in place at a similar facility for a year and received recognition for its success at the American Association of Cardiovascular Annual Meeting in 2016.
