CommuniCare Health Services nursing and rehabilitation centers offer a comprehensive continuum of exceptional care including subacute and medically complex transitional care, rehabilitative services, long-term nursing care, Alzheimer's and dementia care, and outpatient services.

CommuniCare's Specialty Care Centers for persons with dementia or Alzheimer's disease provide safe and supportive residential environments that include a wide range of services to assist the resident in retaining their independence. We design these centers with the Alzheimer's patient in mind, equipping them to provide more specialized services than long-term care centers. Residents can personalize their suites with their favorite things; families are welcome to visit at all hours; specially trained staff use visual and verbal cues to gently remind residents where they are and what is happening; and therapeutic recreation, leisure activities and grooming skills aid the resident in remaining as independent as possible.

Our Specialty Care Centers also include Specialty services for the physical, emotional and developmental needs of infants, children, adolescents and young adults, as well as behavioral units for older adults suffering from emotional diagnoses along with their physical diagnoses.

CommuniCare's assisted living residences offer residents a safe, comfortable, nurturing environment in which independence, privacy, and active lifestyles can be easily maintained. Services include personal care, medication monitoring, 24-hour staffing and security, dining, private suites and social, community and recreational activities.
