Memory Care Services
Our Memory Care program, Connections, is focused on reconnecting our residents to their past and present as well as to the things and people they know and love. Our program goal is to provide maximum quality of life for this population in a safe and secure environment.
The Connections team uses an interdisciplinary approach to care, and designs an individual care plan for each resident, specially geared to their clinical needs and improved quality of life.
Three Foundations of Connections
Preservation – We ensure the individuals past is part of their present – creating Connections.
Purpose – Found in stimulating activities provided to meet the needs of each resident 24/7.
Pace – The flow of the day is patient-centered around the needs of the resident. Comfortable and flexible routines are followed each day.
Features of Connections
- Person-centered programming is offered 24/7 in an effort to enhance the continued independence of the memory impaired population.
- Our program offers dining experiences that promote independence and dignity.
- Quarterly care conferences are attended by the multidisciplinary team to reassess and reevaluate each resident’s unique needs.
- Quarterly family nights are held to bring residents and their loved ones together in a comfortable environment.
