Prior Admission

The process of admission to any skilled nursing home or long-term care facility can be overwhelming. If you desire, one or more members of our friendly team of senior care service professionals will meet you at the hospital or your home to address any questions or concerns you may have about our approach to senior care. We will also provide you with a list of items to bring to the skilled nursing home to ensure that you are prepared to take the next step with us.

Admission

Our admissions coordinator is available to answer questions while you fill out the required forms and discuss the senior care payment options available to you. To aid in the process, we kindly ask that you bring copies of all of your insurance cards at the time of admission. Having these items prepared will allow us to focus on your senior rehabilitation needs appropriately.

Plan of Care

During your stay in our skilled nursing home in Springfield, the interdisciplinary team will meet to discuss the individualized senior care plan for you or your loved one. You and/or your family are always invited to attend these meetings, which address both physical and emotional needs. Progress reports can be provided to personal physicians to ensure continuity of care. Having this level of transparency has allowed us to ensure our skilled nursing and senior care meet the goals of all involved.
